Description
ED Case manager. Must have RN License, hospital discharge planning and case management experience. ED experience a must. AHSV. 4-5 years of Acute ED; Bedside visits not remote; End of life discussions/POLST/Advance Directives/Ethical consultations; Bachelor’s Degree (BSN): Preferred; Experience in a care management role: Required; Case management certification: Nurse (RN) licensure in state of practice: Required.
