April 2026 ICD-10-CM/PCS Code Updates

On April 1, 2026, the ICD-10-CM and ICD-10-PCS classifications will again receive their semiannual update. While this update may appear modest at first glance, there are several changes coding professionals should be aware of.

Here are some highlights of the most important updates impacting coding practice.

ICD-10-CM Updates

For April 2026, there are no code additions, deletions, or revisions to ICD-10-CM. The total number of codes remains 74,719, and there are also no guideline changes. Although this indicates stability in the classification, several revisions to index entries and tabular notes require coder attention.

One notable clarification involves the relationship between Parkinson’s disease and Parkinsonism. A prior index entry linking Parkinson’s disease to Parkinsonism has been removed. Coders should recognize that Parkinsonism is a broader term that may include drug-induced Parkinsonism and other related disorders. Documentation must clearly support the specific diagnosis before assigning a code.

There were also revisions affecting neuroendocrine tumor classification, reinforcing the importance of identifying tumor behavior (malignant, benign, or unspecified) when assigning codes.

Several instructional notes were revised from “Code First” to “Code Also.” This change affects conditions such as neovascular secondary angle-closure glaucoma and hypertensive emergency. The update provides more flexibility in sequencing based on the clinical scenario.

Updates were also made to breast density reporting. Codes R92.31–R92.34 now align with the BI-RADS reporting system by using A–D density categories rather than the numeric 1–4 scale.

Another important change involves revisions from Excludes1 to Excludes2 notes across several categories. This means certain conditions that were previously not reportable together may now be coded together when documentation supports both conditions.

ICD-10-PCS Updates

ICD-10-PCS saw more significant activity in this update with 80 new codes and 2 deletions, bringing the total number of PCS codes to 79,115.

One correction involves stem cell administration. Previously classified as transfusions, embryonic stem cell procedures are now correctly coded as Introduction, reflecting the PCS definition that transfusions involve only blood or blood products.

Several new technology procedures were also introduced. These include innovations such as tissue-engineered vascular grafts for pediatric cardiac conditions, venous flow modulation devices for heart failure, lumenless defibrillator leads for pediatric patients, and investigational intracochlear gene therapy procedures.

Final Thoughts

Even in years with limited ICD-10-CM structural changes, updates to notes, indexing, and procedural coding can still impact coding accuracy and sequencing. The April 2026 update continues to reflect evolving clinical technology while refining coding guidance for existing conditions.

Coding professionals should review these updates carefully to ensure accurate code assignment, compliance, and appropriate reporting.
